Assisted Discharge
I am writing to make a request under the Freedom of Information Act 2000. Specifically, I would like to request information regarding your Assisted Discharge and Support at Home Services.
For clarity, we are referring to those services where a third-party provider offer support, transport and settling services to patients to ensure an efficient, safe discharge which reduces the incidence of delayed or failed discharges. Support at Home services are those where a third party provides a set period of support to patients recently discharged from hospital to increase independence and reduce the incidence of readmission.

We have a number of services who work collaboratively to aid discharge from hospital. These include:
- Bridging Service (Hospital discharge service for ASC eligible users) run by Mid and South Essex NHS Trust
- By Your Side (Home from hospital – low level grant funded service to aid with practical issues e.g. shopping)
- Reablement – run by both internal and externally commissioned home care services

- Bridging - £207k
- By Your Side - £70k
- Reablement – varies based on need but hourly rate for external reablement is £21.54
- What is the duration of the contract/s?
- Bridging – Ends 2024 with the possibility for two further 1 year extensions
- By Your Side – Grant agreement that ends 31st March 2025
- Reablement – Internal (no end date). External 2025
