Recovery from illness
Most people are able to leave hospital and return to the community without needing extra support or services. Other people may need additional advice, support or services to help them.
From the day you're admitted into hospital, the staff involved in your care will work with you to assess the health and social care needs you'll have when you leave the hospital.
Preparing to leave hospital
If you would like to arrange your own care and support, our hospital social work team will be happy to give information and advice.
If you were getting social care support before you went into hospital and there are no changes to your needs, the nurse looking after you will contact our hospital social work team for this to be restarted.
If you were not receiving help before you went into hospital, or you now need more help, you can ask the nurse to refer you to our hospital social work team so they can arrange to assess your needs.
Assessing your needs
An assessment of your needs will include giving advice and guidance to help you:
- regain your independence
- learn new skills and abilities for everyday living
- develop new skills to help with the effects of your illness or injury
- find support and services to help with your on-going needs in the community
Hospital Social Work Team
Our Hospital Social Work Team takes referrals for patients in the following hospitals:
- Basildon and Thurrock University Hospital (BTUH)
- Thurrock Community Hospital
- Mayflower Community Hospital
- Brentwood Community Hospital
You can contact the team from 9am to 5pm, 7 days a week, except Christmas day.
